Insurance claim denial and appeal
Turn every denied claim into a decision and, where it is worth it, an appeal filed before the deadline. An AI agent drafts the appeal from the claim details, the physician signs any clinical justification in the clinical system, and nothing goes to the insurer before the billing manager approves it.
- Duration: 2 to 8 weeks
- How often: For every denied claim
- Steps: 8 steps
- AI agents: 1 AI agent
- Approvals: 1 approval
Free plan, no credit card needed.
What changes with this template
Without it
Denials pile up in the billing inbox until someone has time.
Appeal letters are written from scratch for every claim.
An appeal misses the insurer's deadline by a few days.
The same denial comes back month after month.
With it
Every denial starts a run with its reason and appeal deadline.
An AI agent drafts the appeal, and the billing manager approves it.
The deadline is on the run from day one and checked before anything is sent.
Every run ends with a prevention action and an owner.
The template, step by step
Each step says who does it: someone on your team, an AI agent, or the client through a shared link. An approval keeps every later step locked until it is given.
- Done by your team
- Done by an AI agent
Starts when
The insurer denies a claim. The billing specialist starts a run with the claim number, the insurer, the denial reason and the appeal deadline.
- 1
Log the denial
Day 1Billing specialist · Team
Start the run with the claim number, the insurer, the denial reason from the remittance and the deadline for an appeal. Use the claim number, not the patient's name.
Checklist
- Denial notice saved with the claim
- Appeal deadline checked against the insurer's rules
Fields to fill in
- Claim number (required)
- Insurer (required)
- Denial reason (required)
- Appeal deadline (required)
- 2
Find the cause and choose the action
Days 1 to 3Billing specialist · Team
Compare the claim with the visit record, the coverage and any authorization, and decide whether to correct and resubmit, appeal or write off.
Checklist
- Coverage on the date of service checked
- Codes checked against the visit record
- Authorization checked
Fields to fill in
- Cause of the denial (required)

Sign the clinical justification
Days 2 to 5Treating physician · Team

Checklist
- Corrected codes confirmed
- Letter of medical necessity signed where needed
Fields to fill in
- Reference of the signed letter
- 4
Draft the appeal
Day 5Appeal writer · AI agent
The appeal writer drafts the appeal letter or the corrected claim note from the claim number, insurer, denial reason and cause, in the format in its instructions, and lists the documents to include.

Check the draft against the insurer's appeal rules, the cause and the deadline, correct it and approve it.
Checklist
- Deadline not passed

- Submission confirmation saved
Fields to fill in
- Submitted on (required)

Fields to fill in
- Outcome (required)
- Amount recovered
- Next step if denied again
- 8
Fix the cause
After the decisionBilling manager · Team
Decide what changes so the same denial does not come back, such as a check at the front desk, a coding rule or an authorization step, and who owns the change.
Checklist
- Front desk or coding team told
- Denial type counted for the monthly review
Fields to fill in
- Prevention action (required)
- Owner of the change (required)
Done when
The claim is paid, written off or closed after appeal, and the cause of the denial has a fix and an owner.
Make it yours. Rename steps, change owners and add checklist items, fields or approvals. The template is a starting point for the way your team works.
The AI agents in this template
An agent only works on its own steps, and a person on your team checks what it did. Anything an agent writes to a connected app waits for approval unless you allow it.
Appeal writer
Drafts the appeal from the claim details and the cause the billing specialist found.
What it does
- Reads the claim number, insurer, denial reason and cause on the run
- Follows the appeal format in its step instructions
- Writes the draft appeal and the list of documents into its step
- Completes its step, so the billing manager can review it
Checked by: The billing manager corrects and approves the appeal in step 5.
Questions this template answers
Log the denial with its reason and appeal deadline the day it arrives, then find the cause and decide whether to correct, appeal or write off. Steps 1 and 2 do this, and the deadline is on the run from day one.
Start from a standard format filled in with the claim details and the cause you found. In step 4 the appeal writer drafts the letter and lists the documents to include, and in step 5 the billing manager corrects and approves it.
Record the appeal deadline when the denial is logged and check it again before anything is sent. Step 1 records the deadline, and step 5 is an approval gate where the billing manager confirms it has not passed.
The treating physician, in the clinical record, never the billing team or an AI agent. In step 3 the physician signs the letter of medical necessity in the clinical system, and the run records only its reference.
Fix the cause after every decision, so the same denial does not come back next month. Step 8 records a prevention action and its owner, such as a new front desk check or a coding rule.
Keep every appeal open until the insurer decides, and record what was recovered. Step 7 records the outcome and the amount, and the runs still open show which appeals are waiting.
